About the Plan

Coverage is offered through the State of New Mexico’s benefit program. The State of New Mexico has contracted with ERISA, a third party administrator (ph: 855-618-1800), to manage the medical/prescription and dental programs.

The dental plan is a Preferred Provider Organization (PPO) program which provides coverage at 100% of the scheduled fees for preventative and diagnostic care not subject to the deductible. After meeting a $50 deductible ($150 maximum per family), other dental services are covered at a percentage of the scheduled fees. The maximum paid by the plan, per calendar year, is $1,750 per covered person. Orthodontic coverage is also included up to $2,000 lifetime maximum per covered child and $1,750 lifetime maximum per covered adult.

Dental coverage is effective the first day of the pay period following 30 days of employment.
