Group Basic Life and AD&D

large_family.jpgLife insurance is a valuable tool to provide for our loved ones in the event of premature death. Contributions are made by payroll deduction and the University pays a percentage based on employee salary. The amount of coverage is equal to two times the employee salary, rounded up to the next highest thousand with the maximum benefit being $75,000. This benefit includes an accidental death and dismemberment benefit equal to the basic life insurance policy. The total monthly cost is .382 cents per $1,000 of coverage. Please note that 9-month employee annual premiums are paid over the academic year.

Eligibility

Eligible employees are considered regular status employees and non-regular term appointment employees working 30 hours or more per week. Family members are not eligible for this benefit.

Enrollment

New eligible employees may enroll by completing and submitting the Benefit Enrollment/Waiver Form within their first 31 days of employment. Benefits are effective the first pay period after 30 days of employment. Late enrollment is available at any time, with approval from the carrier. Late enrollees must complete the Benefit Enrollment/Waiver Form and the Evidence of Insurability form. The enrollment form and the Evidence of Insurability form must be sent to Benefits Services. Coverage is effective the first pay period after approval is received from the carrier.
